    National Diabetes Prevention Program (NDPP)

The National Diabetes Prevention Program (NDPP) is a group lifestyle change program geared to support individuals with or at risk for pre-diabetes. This 12-month program focuses on lifestyle changes such as nutrition, physical activity, and stress management. The program is broken down into 6-month core phase and 6-month maintenance phase where participants will meet with their group weekly and then monthly. This program can be offered multiple ways: in-person, virtually, distance or a combination. For distance options, Nebraska has partnered with HabitNu, the online platform for Diabetes Prevention.

Diabetes Self-Management Education and Support (DSMES)

Whether you are newly diagnosed with diabetes or have been living with diabetes for years, support is available.

Diabetes Self-Management Education and Support (DSMES) provides the knowledge, skills, and tools to help you manage diabetes and feel more confident in your everyday health decisions. Through personalized education and support, you can learn to make healthy food choices, stay active, manage medications, monitor blood sugar, and set realistic goals.

    Living Well Programming

Living Well is Nebraska's name for the Self-Management Resource Center (SMRC) evidence‑based self‑management programs originally developed at Stanford University. Program workshops are led by trained facilitators and help participants take small, achievable steps toward managing their health and improving daily life.

These interactive workshops are more than lectures. Groups of up to 14 participants meet for 2–2.5 hours once a week for six weeks to learn practical skills, share experiences, and support one another. Each session provides tools and strategies to help you better manage ongoing health conditions and improve your quality of life.
