Former Nebraska state wards can receive personal records that are kept by the Division of Children and Family Services. Former wards can also request verification letters to show they were a Nebraska state ward. Verification letters are requested for grants, housing, financial aid and other programs.

Former Ward Records Request Form. The form requires a notarized signature and can be submitted via email or mail to the addresses listed on the form.

Former Nebraska state wards may also request a letter listing the dates they were a state ward by:
Providing your full name and date of birth and contacting DHHS through one of the means below:
- Contact your current DHHS caseworker, if you have one;
- Send your request to DHHS.ChildrenandFamilyServices@nebraska.gov with the subject line “Former Ward Verification Letter"; or
- Call (402) 471-3121 to make this request.
