Preventive Health and Health Services Block Grant - Chronic Disease NWS-03
The purpose of this funding opportunity is to support activities that expand access to, awareness of, and participation in chronic disease prevention and health promotion initiatives that contribute to obesity prevention amongst adults. Activities may include education, training, outreach, communication strategies, evidence-based programs, community engagement, and other innovative approaches designed to improve health outcomes and promote healthy behaviors. Proposed activities should complement, enhance, or build upon existing programs and services and should not duplicate current federally funded efforts.
