This form is used to order WIC materials from the state WIC office. It includes materials purchased from outside vendors and materials that the state prints for the WIC agency. You may place orders at any time, but they will be shipped out quarterly. Please order the amounts you anticipate using for a 3-month period. If a material is not on this order form, it is not available to be ordered from the state at this time.
This order form is for use only by WIC staff. If you do not work for WIC, please contact a local WIC clinic or the state WIC office at 402-471-0158 to request materials.
All orders will be sent to the address we have on file for the local agency selected. If you have any questions, please email DHHS.NebraskaWIC@nebraska.gov.
