Step 3:  Important:  Begin gathering required documentation for each adult and child applying for WIC that shows proof of identity, income, and address for every person applying. Examples of acceptable documentation are listed in the next section. Please contact your WIC clinic if you have questions or difficulty accessing this documentation.

You must bring proof of identity for every adult and child applying for WIC. Bring it to every visit. If you cannot bring the required information, you must contact the WIC office.

Examples:

• Driver's license, state issued ID, passport (with photo)
  
• Medicaid card
• Social Security card
  
• Birth certificate
  
• Foster placement papers or custody documents (if applicable)
  
• Purple WIC ID Card (if re-certifying)
• Immunization records​
  
• Notice of Action - if listed as an eligible participant in Medicaid, SNAP, ADC, 599CHIP
  

• You must bring proof of Nebraska address for anyone applying for WIC. 
• Documentation must be postmarked or dated within the last 30 days. ​
• For Women - mail with woman's name or spouse's name
• For Child - mail with child's name or legal guardian's name ​
• If you cannot bring the required information, you must contact the WIC office.

Examples:

• Physical mail or envelope showing name and street address 
• Physical mail showing the service address
• Utility bill, cable bill, lease, property tax statement, rent or mortgage receipt 
• Online mail showing name and address or service address
• Note: Street address must be included; mail with only a P.O. box is not allowed​​

Bring all proof of income for everyone in the household received within the last 30 days 

If you are currently receiving Medicaid, SNAP, or TANF, bring proof of your participation:

• DHHS Notice of Action Form
  
• Medicaid, SNAP, TANF cards
  

If you are not currently receiving Medicaid, SNAP, or TANF, bring proof of all incomes sources for the last 30 days including:

• Pay stubs 
• Self-employment documents: W-2 forms; most recent income tax statement; accounting records
• Military LES - Leaving & earning statement
• Alimony or child support statements
• Unemployment or disability income
• Social security statements (award letter or bank statements)
• Income letter from your employer if paystub is not available
• Retirement, pensions, annuity payments including payments for veterans
• Disability income payments including veteran's disability payments
• Workers compensation benefits
  
• Net royalty payments; lottery winnings or prize money over $1,500
• Inherited cash
• ​Notice of Action - if listed as an eligible participant in Medicaid, SNAP, ADC, 599CHIP
• ​Other income sources, contact your clinic​
  

• You'll receive an overview of WIC services and benefits.
• Staff will review your Rights and Responsibilities in the WIC program - English; Spanish
  
• Your documentation (ID, income, residency) will be reviewed; missing items may require rescheduling.
• Staff will check income eligibility and assess any nutrition risk factors for you and your children.
• Nutrition and health questions will be asked for each applicant. ​
• Height/length, weight, and hemoglobin may be measured.
  
• Nutrition and breastfeeding education and referrals will be provided as needed.
• Staff will help you set nutrition or breastfeeding goals for your next visit.
• WIC food benefits for eligible family members will be loaded onto your Nebraska eWIC card for immediate use.
• Staff will review your food benefits, explain how to shop for WIC foods, and help you set up the WICShopper app.
• You'll schedule your next appointment before leaving.​​
