This page is a resource to Practitioners and Health Care Organizations who wish to exchange healthcare data with Nebraska DHHS, those participating or wishing to participate in the Merit-Based Incentive Payment System (MIPS) Promoting Interoperability Performance Category and for eligible hospitals and critical access hospitals (CAHs) participating in the Medicare Promoting Interoperability Program (PIP) or needing information on Reportable Conditions Reporting.

Healthcare data exchange, reporting and interoperability ultimately improve patient care when shared appropriately between healthcare and public health partners. 

Hospitals, and CAHs must select an EHR reporting period of a minimum of any continuous 90-day period in the calendar year. Changes in the Final Rule for 2022 are reflected in the Federal Registry

Steps for Participation

If you are an eligible health care entity or provider who is currently participating with CyncHealth through the sharing of clinical information pursuant to Neb. Rev. Stat. § 81-6,125, please contact to confirm you are meeting CMS requirements.

Step 1

If your facility has never exchanged data with the state OR your facility wants to initiate exchange of additional types of data: Complete the registration form to select the data sets your organization is interested in. You can preview the form here​. ​​​Once all information is gathered it will likely take approximately 10 minutes to complete. You are not able to save and edit the online form but it can be started over if necessary.​

DHHS will contact you to discuss next steps​.
