This page is a resource to Eligible Practitioners and Health Care Organizations participating or wishing to participate in the Merit-Based Incentive Payment System (MIPS) Promoting Interoperability Performance Category and for eligible hospitals and critical access hospitals (CAHs) participating in the Medicare Promoting Interoperability Program (formerly Meaningful Use) or needing information on Reportable Conditions Reporting.

Healthcare data exchange, reporting and interoperability ultimately improve patient care when shared appropriately between healthcare and public health partners. 

Hospitals, and CAHs must select an EHR reporting period of a minimum of any continuous 90-day period in the calendar year. Changes in the Final Rule are reflected in the ​Federal​ Registry.   ​​

Steps for Participation

Step 1

If your facility has never exchanged data with the state OR your facility wants to initiate exchange of additional types of data: Complete the registration form to select the data sets your organization is interested in.

DHHS will contact you to discuss next steps​.
