National Diabetes Prevention Program (NDPP)

The NDPP is a CDC-recognized, evidence-based lifestyle change program. It is designed for people who have prediabetes or are at risk for type 2 diabetes, but who do not already have diabetes. To know if you are at risk for type 2 diabetes complete the prediabetes risk test.

A trained lifestyle coach leads the program to help participants improve their lifestyle, including eating healthier, reducing stress, and getting more physical activity. The program is offered in person or online and includes group support from others who share your goals and struggles.​​

Diabetes Self-Management Education and Support (DSMES)

DSMES is an evidenced based approach to empowering people with diabetes to navigate self-management decisions and activities. ​

DSMES is provided by diabetes educators who are experienced healthcare professionals. Educations sessions provide patients with diabetes information​ about collaborating with their health care team, making informed decisions, solving problems, developing personal goals and action plans, and coping with emotions and life stresses.​

Health Coaches for Hypertension Control Program (HCHC)

HCHC is an evidenced-based program developed by Clemson University for individuals with high blood pressure or at risk for developing high blood pressure. 

​This 8-week program is led by health coaches who teach participants how to properly measure their​ own blood pressure and improve risk factors of high blood pressure including healthy eating, tobacco cessation, physical activity, medication management and stress management.

Living Well Chronic Disease Self-Management Programs (CDSMP)

Living Well is a 6-week evidence-based workshop licensed through​​h the Self-Management Resource Center​. The 6-week workshop is designed for anyone with a physical or mental health condition or their caregivers.

Trained Living Well leaders educate participants about action planning, communication, healthy eating, physical activity, medication management, and more to assist participants in taking small steps to improve​​ their health. ​

There are four Living Well programs offered in Nebraska: 

  1. ​Living Well with Chronic Conditions
  2. Living Well with Diabetes 
  3. Worksite Living Well
  4. Living Well with Chronic Pain​​
