Health Coahces for Hypertension Control (HCHC) is an 8-week evidenced-based program developed by Clemson University for individuals with high blood pressure or at risk for developing high blood pressure.
This program is led by health coaches who teach participants how to properly measure their own blood pressure and improve risk factors of high blood pressure including healthy eating, tobacco cessation, physical activity, medication management and stress management.
