​​​​​​​​​​​​​​​​State Health Assessment (SHA)

Every five years, the Division of Public Health (NE DPH), within the Nebraska Department of Health and Human Services (DHHS), leads a collaborative process, which result in data-driven state-wide examinations of the health and wellbeing of the communities across Nebraska. This process results in a state health assessment (SHA) which serves as a foundation for a state health improvement plan (SHIP). To create the SHA, a wide variety of data was drawn upon. This includes qualitative assessments gathering input from Tribal and Local Health Departments (T/LHD) and community partners, as well as quantitative public health data sources.

The 2022-2023 SHA helps to identify factors, gaps, trends, and progress in both the public health system and the health status of Nebraskans. It includes selected public health system priorities and health status priorities based on assessments and the collection of data. The selected priorities will help inform the goals, objectives, and strategies contained in the Nebraska 2023-2027 SHIP.

Vision:  Working together to improve the health and quality of life for all individuals, families, and communities across Nebraska.

Mission: Serving as a blueprint for action, the Nebraska Division of Public Health uses the SHIP to advance public health priorities by addressing alignment and coordination efforts at a systems level. Using a collaborative planning approach, Nebraska's Division of Public Health systematically engages public health partners, including Tribal and Local Health Departments (T/LHD), to foster shared decision-making ownership and responsibility for the plan's implementation. 

The following resources are available to assist the state and its partners with the overall SHIP process.  

  • Prioritization Guide – This thorough guide provides the state and its partners with a systematic approach to determining the top health status priorities at the state level. Key findings from an evaluation of the SHA/SHIP process serve as the foundation for the recommendations within the guide, with relevant samples, resources, and thorough discussion accompanying each recommendation.
  • SHIP in Action – Designed with the state and its partners as the target audience to guide the overall SHIP process from the beginning to the point of putting the pieces into action.
  • CHA/CHIP Toolkit – Designed as a comprehensive toolkit for Nebraska Tribal and Local Health Departments (T/LHDs), as well as other organizations conducting community needs assessments and planning. The toolkit includes both a FAQ and supplemental Resource Inventory which work together to provide guidance and resources for common questions that Nebraska T/LHDs may have when doing their Community Health Needs Assessment (CHA) and Community Health Improvement Plan (CHIP).
