What you need to know

​​The Additional Information Request form is to be used to provide information to the Office of Consumer Affairs and to be used by applicants and Certified Peer Support Specialists under the following circumstances:​

  • ​​To report a misdemeanor or felony while certified as a CPSS
  • To report any change in curriculum content or curriculum trainers by a State of NE approved  Peer Support Core Curriculum Provider
  • To provide any additional information requested by the Division of Behavioral Health, Office of Consumer Affairs related to any application submitted for approval
