  • Initial License: $625
  • Renewal: $625
  • Duplicate/reissued license: $10
  • Administrative fee $25 (for a denied or withdrawn application)
  • Certification for a facility: No Fee

License Renewal
Licenses are renewable every year on July 1.
 
Closing Information

THERE IS NO FEE FOR CLOSING A DISPENSING PRACTITIONER LICENSE.

If you are changing location, complete a closing form and apply for a new license.

If you are closing, complete the form below within 15 days after closing. A closing controlled substances inventory is NOT required.
