What is changing?

​The new feature available via the Nebraska State Immunization Information System (NESIIS) gives Nebraskans the ability to download their SMART Health Card/Immunization Records to an iOS or Android smartphone using a QR code. ​

How do I download my immunization records on an iOS device?

​Instructions for downloading your immunization records on your smartphone can be found on the NESIIS Resources website. Check out the Training Resources page​ for an instructional video and quick reference guide. 

What if I don’t have a Smartphone?
NESIIS provides the option to download your immunization records to a computer and print them.
What information do I need to provide in order to access my immunization records?

You will need to enter your first name, last name, date of birth, and social security number into NESIIS to access your immunization information. ​

Do I need to download an additional app on my phone?
If you have an Android device, you will need to download the CommonHealth app from the Google Play store. If you have an iOS device, the Health app is already available on your phone.
Why do I see two QR codes on my records page?
If you have more than 25 entries on your immunization record, the system will automatically generate two QR codes. The first code will download the first 25 records, the second will download the remaining records.
How do I download all of my immunizations at once?

​Use the Export Full Immunization History option on the NESIIS screen to export your full immunization history. ​

Can I also download my dependent child’s vaccination records?
Yes, you would just enter your child’s information instead of your own in NESIIS.
How do I print my records?

​Once you have accessed your immunization records, click Print at the top of the page to download and print your entire immunization history. 

Can I download my records again if I lose my phone, or need them on a different device?
Yes, you can download your immunization records on as many devices as you need.
What are the benefits of this new feature?

Here are some of the benefits of the new immunization download feature:

  • Mobile accessibility to immunization records via a QR Code.
  • An option to print full immunization records. 
  • A streamlined process for schools, travel authorities, and healthcare organizations to verify immunization records quickly and efficiently.​


What if I am missing some immunization records?

​All questions/inquiries regarding your immunization records should be directed to your healthcare provider.​

I am having trouble scanning the QR code on my Android phone. What should I do?

​If you are using an Android phone, move your phone closer to the screen to get the box smaller and ensure it is lined up with the QR Code.​
