The DHHS Accommodation Services website provides Nebraskans who require accommodations while seeking DHHS services with a questionnaire they can complete in order to obtain referrals to the DHHS team that can best meet their most critical need. Nebraskans, or someone on their behalf, such as a community partner, can access and complete the questionnaire, after which they will receive a Survey ID and contact information for the team that can best meet their needs.
