What you need to know

​​ Certificate of Need

Q. How do I obtain a Certificate of Need?

A. Please visit the Certificate of Need section on the Hospital Licensing home page for details.

Changes

Q. Does changing location void the license or can we use our existing license number once we've moved?

A. You cannot use your current license number if you move. You cannot provide services at a new location until you have a new license. Contact us for a new license application.

Q. The Administrator/CEO changed at the facility; what do we need to do?

A. Notify us in writing of that change within 5 days.

Q. We want to build on to our existing licensed facility; what do we need to do?

A. Notify us in writing before any new construction that affects patient care and treatment areas of your facility. Please see the facility construction website.

Q. We want to change the number of patient beds at our hospital; how do we do that?

A. Notify us in writing of this change before the date of the change. We will notify you if we need any other documentation once we've received and reviewed your request.

Q. We would like to add services to our hospital; what do we do?

A. Notify us in writing of the new services and what date they will start.

Fees

Q. Does the Department accept credit card payments for fees?

A. No, we do not accept credit card payments. 

General

Q. Can the Department use a different mailing address other than the preferred address listed on our application?

A. You can use a different address than the one on the application only if you notify us in writing of the different address.

Licensure, Certification and Accreditation

Q. How do we get a license for a new hospital?

A. Please contact us for an initial hospital licensure packet. See the regulations here​​ to see what documents you will need to submit.

Q. How long is my hospital license effective?

A. Hospital licenses expire annually at the end of December. The license is valid until the renewal date.

Q. Who can sign the licensure application forms when the owner is a corporation?

A. Only the corporate officers (President, Vice President, Treasurer and/or Secretary) can sign the licensure application (a corporation's Board member/Board officer cannot sign as a corporate officer).

Occupancy Certificate

Q. How do we obtain a Certificate of Occupancy?

A. Notify us of the location needing the occupancy permit. We will submit a referral to the State Fire Marshal's office or to the delegated authority on your behalf.

Offsite Locations and Additional Services

Q. We want to add an offsite/satellite location to our hospital; how do we do that?

A. Notify us in writing of your request to add the new location to your license. We will notify you if we need any other documentation once we've received and reviewed your request.
