Health clinic licenses expire on February 28 every year and must be renewed annually.

An applicant must meet the requirements in Title 175 Chapter 7, Regulations Governing the licensure of Health Clinics and in the Health Care Facility Licensure Act. These requirements include:
Submitting a written application
Submitting the required documentation and fee(s); and
Passing an onsite inspection prior to patient use and occupancy
Note:
- A health clinic may not operate until the inspection has been completed and the Department determines a license can be issued.
- A change in location of the facility or a change in ownership of the facility voids the current license and requires a new license to be issued prior to providing patient care and services.

Initial Licensing Fees:
- All types of health clinics except public health clinics and ambulatory surgical centers: $600
- Public health clinics: $400
- Ambulatory surgical centers:
a. One operating/procedure room: $1,250
b. Two to three operating/procedure rooms: $1,350
c. Four or more operating/procedure rooms: $1,450
Renewal Licensing Fees:
- All types of health clinics except public health clinics and ambulatory surgical centers:
a. One to 50 patient admissions in the past year: $600
b. 51 to 100 patient admissions in the past year: $800
c. 101 or more patient admissions in the past year: $1,000
- Public health clinics:
a. One to 50 patient admissions in the past year: $400
b. 51 to 100 patient admissions in the past year: $450
c. 101 or more patient admissions in the past year: $500
- Ambulatory surgical centers:
a. One operating/procedure room: $1,250
b. Two to three operating/procedure rooms: $1,350
c. Four or more operating/procedure rooms: $1,450
d. All ambulatory surgical centers must also pay an additional fee under the Outpatient Surgical Procedures Data Act, Neb. Rev. Stat. §§ 81-6,111 to 81-6,119, as follows:
500 or fewer outpatient surgeries per year: $275
501 to 2,000 outpatient surgeries per year: $350
More than 2,000 outpatient surgeries per year: $425

Refunds for denied applications:
- If the Department did not perform an inspection, the license fee is refunded except for an administrative fee of $25; or
- If the Department performed an inspection, the fee is not refunded.
