  • Documentation of any misdemeanor or felony conviction(s) since the date your current CPSS credential was issued, whether the conviction(s) occurred in Nebraska or another state. You can find the date you were issued your certification on your certificate OR at the license information lookup websiteThe documentation must include the following:
      • List the date of conviction(s), the county and state in which the conviction occurred, and the type of conviction(s). Please include a brief description of the conviction(s) including what the conviction was for, what happened, and who was involved.

      • If the applicant is currently on probation, a letter from the applicant's probation officer addressing the terms and current status of the probation.

Please note that additional information may be requested upon initial review of your application.

All applicants are required to complete the Adult and Child Protective Services (APS/CPS) registry and pay the associated fees as outlined on the application. Previously completed APS/CPS checks cannot be accepted. All applicants must complete a new APS/CPS check for CPSS recertification.

      • NOTE: Upon completion of the APS/CPS registry check, you will be provided a request/confirmation number that will be a REQUIRED entry on the CPSS Application before you can submit the application to the OCA for approval.
      • If you prefer to complete the Adult and Child Protective Services Registry Check online there is a link on the instruction page of the CPSS Recertification Application itself.
      • If you prefer to complete the Adult and Child Protective Services Registry Check via paper form, please print the form from the Central Registry Forms page and complete it according to application instructions. You will need to enter the following number on the application so that the information is then provided to the DBH OCA: 88782218.

You will not need to upload any certificates of completion for the CEUs, but you will need to attest to having completed them. It is suggested that you have the CEU completion documentation available for review by the State of Nebraska Division of Behavioral Health, if you are randomly selected for an audit.
