NOTE: The designated manager is responsible for all transactions conducted at the establishment, in compliance with the statutes, rules and regulations relating to funeral directing and embalming and establishments.

Branch Establishment
- Be a subsidiary of a licensed funeral establishment
- Employ as its manager a licensed funeral director and embalmer who is responsible for all transactions conducted in the branch establishment (exception: any person holding a valid license as only a funeral director may serve as a manager of a branch establishment)
- Pass an on-site inspection.
Funeral Establishment
- Employ as its manager a licensed funeral director and embalmer who is responsible for all transactions conducted in the funeral establishment (exception: any person holding a valid license as only a funeral director may serve as a manager of a funeral establishment)
- Pass an on-site inspection.
Crematory
- Location must meet local zoning, building and environmental regulations
- Send a certificate showing the operator was trained by the Cremation Association of North America or by the maker of the cremation chamber
- Meet operational and physical plant standards
- Complete a self-inspection report.
Change in Location: Submit a NEW application to the Department at least 30 days prior to the change.
Change in Manager: Submit an application to the Department at least 15 days prior to the change.
Change in Name: Submit an application to the Department at least 30 days prior to the change.
Change in Owner: Submit an application to the Department at least 15 days prior to the change.
Change in Services Provided or Adding a Prep Room: Notify the Department of the establishment's intent to have a preparation room; and must have received a successful inspection rating within 30 days of the anticipated change in services.
