Emergency Care Provider Licensing

The State of Nebraska requires all Emergency Medical Responders, Emergency Medical Technicians, Advanced Emergency Medical Technicians, and Paramedics to be licensed.

Individuals who are licensed as an Emergency Medical Technician-Intermediate may renew and reinstate only. 

Applications and supporting documents may be sent scanned and sent electronically to email address listed on the application.  Application may be completed and signed electronically.

The fee for reinstatement of a license is $35.00.

Temporary Licensing Application:

Temporary Emergency Care Provider

Military Spouse Temporary Credential:  If you have an active credential in another state and you are a military spouse, you may be issued a temporary credential pending completion of the permanent credential requirements. A temporary credential specifically for military spouses is available under Neb. Rev. Stat. §38-129.01 and is issued for a period not to exceed 1 year. Review the requirements in both the initial and temporary license application to determine which process is right for you.

EMS Service Licensing & Inspection

The State of Nebraska licenses Emergency Medical Services as Basic Life Support and Advanced Life Support. Services can apply for either a transport or a non-transport license.

Renewal Information:

Refer to Title 172 NAC 12-004 and 12-007 for service renewal requirements.

Inspections: 

EMS Services will be inspected at least once on a triennial basis.  Your Regional EMS Specialist will be contacting your service when it is due for inspection.  Please feel free to contact your Regional EMS Specialist if you have questions.  

The Office of Emergency Health Systems is providing the following Guidance Document to assist you in preparing for your inspection.  Additional documents can be found below to assist you with your inspection.

For services with multiple sites, you will be asked to complete the Mutlitple Site Form.

Certification and Verfication

Verfication information may be obtained by using Licensure Lookup System.

To request a certification of your license be sent directly to another state licensing agency, send a written request to our office (address is below) and include the following:

  • Your full name as it appears on your license;
  • Your profession;
  • The address to send the certification to; and,
  • The proper fee – check or money order made payable to DHHS EMS Licensing.

Certification Fee - $25.00 

Certification includes:

  • Name;
  • Profession;
  • License number;
  • Issuance date;
  • Expiration date;
  • Whether the license was granted by examination, waiver, or reciprocity;
  • Examination information, if available;
  • Any disciplinary action taken against the license with the type(s) and date(s) of the action if applicable; and
  • SEAL of the State of Nebraska Department of Health & Human Services.
