The Division of Public Health's Obesity, Nutrition, and Activity Initiative uses state and federal funds to work with community partners to prevent obesity and its risks. The program promotes healthy eating, physical activity, and education to create spaces that support both mental and physical wellness.

By focusing on overall well-being through education and support, we help people make better choices and build a healthier future.

Heritage Health

Nearly all Medicaid and CHIP enrollees receive their physical health, behavioral health, and pharmacy benefits through a Heritage Health plan. Heritage Health is referred to as managed care. Managed care is a system in which the State contracts with a managed care organization (commonly referred to as an MCO or a health plan) to provide health care benefits and services to Medicaid and CHIP enrollees. The name of your assigned health plan can be found on your Medicaid ID card. You can also call one of the phone numbers listed to find out which health plan you are enrolled with.
