Nebraska's Medicaid managed care program,
Heritage Health, combines Nebraska Medicaid's physical health, behavioral health, dental, and pharmacy programs into a single comprehensive and coordinated program for the state's Medicaid and Children's Health Insurance Program (CHIP) enrollees. Heritage Health members enroll in one of three statewide health plans to receive their health care benefits.
Included in this section is general information on the Heritage Health program, as well as information on the three health plans in the program.

In June 2021, the Centers for Medicare and Medicaid Services (CMS) released a Medicaid Information Bulletin (CIB) notifying states of a new reporting requirement on their managed care programs and operations. The Managed Care Program Annual Report (MCPAR) required in 42 CFR § 438.66(e), consists of the Medical Loss Ratio (MLR) Summary Report required in 42 CFR § 438.74(a), and the Access Standards Report required in 42 CFR § 438.207(d) and (e).
