What you need to know

​​​​​​Coalitions and Learning Collaboratives are groups of community partners with a vested interest in advancing health outcomes in their community. These groups connect clinical and community partners to work towards a shared goal of greater health for all members of the community. 

Cardiovascular Health Learning Collaboratives​​

The Cardiovascular Health Learning Collaboratives (LCs) connect healthcare professionals and community partners to share best practices and implement changes based on community-wide cardiovascular health data. They meet monthly and follow the PDSA (Plan-Do-Study-Act) rapid cycle improvement process to implement changes within cardiovascular health. Any community members or organizations interested in advancing heart health are welcome to join.   ​

Four Corners District

- Butler, Polk, Seward, and York counties

Current work includes partnering with food banks and faith communities to offer nutrition education and supporting community health workshops. 

North Central District

- Antelope, Boyd, Brown, Cherry, Holt, Keya Paha, Knox, Pierce, and Rock counties

Current work includes supporting referrals for the Health Coaches for Hypertension Control (HCHC) program and promoting physical activity resources in each community. 

Southeast District

- Johnson, Nemaha, Otoe, Pawnee, and Richardson counties

This group is currently working to develop an action plan.

​West Central District

- Arthur, Hooker, Lincoln, Logan, McPherson, and Thomas counties

This group is currently working to develop an action plan.​

Falls Free Older Adult Falls Coalition

​The Falls Free Nebraska Older Adult Falls Coalition meets bi-annually to discuss the current state of Falls Prevention work in Nebraska and share resources. The Falls Prevention Program is coordinated by the Chronic Disease Prevention and Control program and support four evidence based falls prevention programs; SAIL, Stepping On, Tai Chi for Falls Prevention, and Bingocize. The Falls Coalition is open to any community members or organizations with an interest in Falls Prevention. Nebraska Comprehensive Cancer Control Program Group

The Nebraska Comprehensive Cancer Control​ program hosts large group meetings quarterly and workgroup meetings targeting the Cancer Plan objectives.
