Application Procedures and Requirements

PHARMACIST

Application for License to Practice Pharmacy

EXAMINATION - requires the applicant to have a Completed Application on file with the Nebraska Licensure Unit and successfully complete the NAPLEX and MPJE Examinations with scores of at least 75.

NOTES:
Nebraska cannot authorize you to sit for exams until you have first registered to take the exams at www.NABP.net.  Once all requirements are received Nebraska will make you eligible to test; you will receive an authorization code from Pearson Vue so that you may go back online to choose your time, date and place to take your exams.

Foreign Pharmacy Graduates - Must obtain the Foreign Pharmacy Graduate Examination Certification (FPGEC) through NABP before seeking licensure in Nebraska.
 
SCORE TRANSFER - requires the applicant to have a completed application on file with the Nebraska Licensure Unit, a NAPLEX score transfer report from NABP with a passing score of at least 75 and successfully complete the MPJE with a score of at least 75.

RECIPROCITY - requires the applicant to have a Completed Application on file with the Nebraska Licensure Unit, submitted a NABP official application for transfer of pharmacist license to Nebraska and successfully complete the MPJE with a score of at least 75. If you are reciprocating from California AND took the NAPLEX prior to January 1, 2004, please call our office at (402) 471-2118 for further details.

PHARMACY TECHNICIAN

Application for Registration as a Pharmacy Technician 

All persons employed as a Pharmacy Technician must be registered prior to the first date of performing the functions of a pharmacy technician.  As of January 1, 2017, if you were registered as a Pharmacy Technician as of January 1, 2016, you must be able to demonstrate completion of a board approved state or national pharmacy technician certification program. If you were first registered after January 1, 2016, you must be certified within one year from the registration date.

Affidavit of Pharmacy Hours
Affidavit of Clinical Pharmacy Hours
Application for Reinstatement as a Pharmacy Intern
