Immunization Program : Adolescent Immunization

By the time a child is 13, he or she should have finished the childhood series of shots. Check immunization records to see if these immunizations have been received:

5 vaccinations

Diphtheria, Tetanus, Pertussis (DTP or DTaP)

4 vaccinations

Polio (OPV or IPV)

2 vaccinations

Measles, Mumps, Rubella (MMR)

3 vaccinations

Hepatitis B

2 vaccinations

Hepatitis A

2 vaccinations


In addition, adolescents also receive:   1 Tdap, 2 Meningococcal, 3 Human Papilloma Virus immunizations.  

If some of the immunizations are missing, talk to your doctor or clinic about finishing the series.   The series will not need to be restarted.   The number needed depends on the age that the immunizations were started.   Although vaccinations may hurt for a short time, they protect from worse pain and harm later.


In 2008, Nebraska started a statewide immunization database (NESIIS), but it is still important for you to keep a record of all your immunizations for yourself.   The immunization should be entered by name long with the date you received them, and the name of the clinic or doctor’s office that gave them to you.   This record gives health care providers the information they need to know so they can treat you appropriately in the future.
