Athletic Training
Requirements for Licensure

You must submit the following:

A. Completed application;
B. Licensure fee;

And include the following:
  1. Age:  Evidence of being at least nineteen years of age:  You must submit a copy of one of the following:
     a.  Driver’s License;
     b.  Birth Certificate;
     c.  Marriage license that provides date of birth;
     d.  U.S. Passport;
     e.  Military Identification; or
     f.  Other similar documentation.

  2. Citizenship, lawful permanent residence, and/or immigration status information:  You must submit a copy of at least one of the following documents:
    a. A U.S. Passport (unexpired or expired):
    b. A birth certificate issued by a state, county, municipal authority or
        outlying possession of the United States bearing an official seal;
    c. An American Indian Card (I-872);
    d. A Certificate of Naturalization (N-550 or N-570);
    e. A Certificate of Citizenship (N-560 or N561);
    f. Certification of Report of Birth (DS-1350);
    g. A Consular Report of Birth Abroad of a Citizen of the United
        States of America (FS-240);
    h. Certification of Birth Abroad (FS-545 or DS-1350);
    i. A United States Citizen Identification Card (I-197 or I-179);
    j. A Northern Mariana Card (I-873);
    k. An Alien Registration Receipt Card (Form I-551, otherwise known
       as a “Green Card”);
    l. An unexpired foreign passport with an unexpired Temporary I-551
       stamp bearing the same name as the passport;
    m. A document showing an Alien Registration Number (“A#”); or
    n. A Form I-94 (Arrival-Departure Record).

    ** A copy of a Certified Birth Certificate or United States Passport will meet the documentation requirement for both Proof of Age and Evidence of Citizenship.

  3. Other Credentialing Info:  If you hold or have held a credential to provide health services, health-related services, or environmental services in Nebraska or in another jurisdiction, you must have the licensing agency submit to the Department a certification of your credential;

  4. Disciplinary Actions:  If you have had any disciplinary actions taken against your credential, you must submit a copy of the disciplinary action(s), including charges and disposition;

  5. Conviction Information: If you have been convicted of a felony or misdemeanor, you must submit:
     a. A list of any misdemeanor or felony convictions;
     b. A copy of the court record, which includes charges and
         disposition;
     c. Explanation from the applicant of the events leading to the
         conviction (what, when, where, why) and a summary of actions
         you have taken to address the behaviors/actions related to the
        convictions;
     d. All addiction/mental health evaluations and proof of treatment, if
         the conviction involved a drug and/or alcohol related offense and
         if treatment was obtained and/or required; and
     e. A letter from the probation officer addressing probationary
         conditions and current status, if you are currently on probation;
     f. Any other information requested by the Board/Department.

  6. Education
    a.  An official transcript (official means issued and sealed by the
         issuing Institution) showing completion of athletic training
         curriculum that is accredited by the Commission on Accreditation
         in Athletic Training Education in conjunction with the Council for
         Higher Education Accreditation; OR
    b. An official transcript from an accredited college of university
        accredited by North Central Association of Colleges and Schools
        or a comparable regional accrediting body with a 4-year degree
        and documentation (Verification of Student Athletic Training form)
        of at least 2 consecutive years of student athletic training military
        duty excepted, under the supervision of an athletic trainer
        approved by the Board.  In order for the supervising trainer to be
        approved by the Board, the trainer must have been overseen:
             1. Under the supervision of an athletic trainer who was
                 licensed in Nebraska; OR
             2. An athletic trainer in another state who met Nebraska
                 licensure requirements at the time the student training was
                 performed         
        
  7. Official documentation of passing the Board of Certification Examination.  Scores must be sent directly from the Board of Certification.  You may contact them at www.bocatc.org and request that certification be sent electronically.

  8. Verification of Student Athletic Training from the Supervising Athletic Trainer if applicable (four-year degree other than athletic training).

  9. Documentation of continuing competency if you have passed the licensure examination but are not practicing.  Must submit proof that within three years preceding the application for licensure you have:
    a. Completed 25 hours of continuing education pursuant to 172
        NAC 17-004; and.  NAC 17-004; and.
    b. Holds a current CPR certificates from a nationally recognized
        organization that issues these certificates; OR
    c. Holds a current certification from the Board of Certification for
        Athletic Trainers (BOC); OR
    d. Has retaken and passed the licensure examination with a
        passing score.

  10. Documentation of continuing competency if you are licensed in another state but are not practicing Must submit proof that within three years preceding the application for licensure you have:
    a. Completed 25 hours of continuing education pursuant to 172 NAC
        17-004; and.
    b. Holds a current CPR certificates from a nationally recognized
        organization that issues these certificates; OR
    c. Holds a current certification from the Board of Certification for
        Athletic Trainers (BOC); OR
    d. Has retaken and passed the licensure examination with a passing
        score.
