The Department investigates complaints regarding Long Term Care Facilities. To register complaints, contact the Licensure Unit, Health Care Facilities and Services Complaints.

Facilities self reporting an incident may use their own report, or one of the following forms:
Abuse, Neglect, or Misappropration
Elopement
Resident to Resident
Accidents
Injury of Unknown Origin
