Communicable Disease Reports

Physicians, hospitals, and laboratories are required by law to submit reports of communicable disease and other situations that pose a threat to the public health.  The following pages provide information regarding reporting requirements and include report forms that can be printed and used to submit reports.  The complete Communicable Disease Regulations are available online at http://www.dhhs.ne.gov/reg/t173.htm.
