Under the umbrella of services and brand name of the Every Woman Matters Program, the WISEWOMAN (Well-Integrated Screening and Evaluation for Women Across The Nation) Program in Nebraska provides screening for risk factors of heart disease, stroke and diabetes to women ages 40-64 who are also clients of the Every Woman Matters Program. In addition to screening, education is also incorporated to reduce the risk of cancer and chronic diseases. Funding is received from the Centers for Disease Control and Prevention, Division of Heart Disease and Stroke.

Data Collection Method and Availability
The data have been collected on a daily basis since 2002 and contains demographics, cardiovascular disease screening records (family history, smoking status, BMI, lipid panel, blood pressure, glucose, A1C), lifestyle intervention records, provider contacts, and claim information. Only aggregate and unidentifiable data are available.
