| Participating practices were limited to the specialties of Family Practice, General Practice, Internal Medicine or Pediatrics in the communities of Scottsbluff, Sidney, Ogallala, North Platte, McCook, Lexington, Kearney, Holdrege, Hastings, Grand Island, Columbus and Norfolk. |
The Nebraska Department of Health and Human Services (DHHS), in collaboration with the Medical Home Advisory Council, published a Request for Interest in July 2010 to solicit practices interested in participating in the two-year pilot.
