Nebraska's Money Follows the Person Project (NMFP)
Am I eligible to transition?
NMFP is available to individuals who meet the following eligibility requirements:
You must currently be residing in a nursing home or ICF/MR (Intermediate Care Facility for Persons with Mental Retardation) a minimum of 90 days (a consecutive stay in a hospital and a facility may also meet this requirement)
You must currently be receiving Medicaid benefits for facility care
You must meet the eligibility requirements for the Aged and Disabled Waiver or one of the Developmental Disabilities Waivers
You must move into a qualified residence:
Personal or family residence
Apartment (with kitchen, bath, and sleeping areas, and lockable entrance)
Community group homes for the developmentally disabled (four or less persons)
To apply for NMFP you may contact the NMFP project office. In addition, you may contact your facility social worker / discharge planner who can assist you with a referral to NMFP.
Upon receipt of your referral or inquiries, a NMFP Transition Coordinator will contact you to discuss the possibility of transitioning to community-based services.
