Transition Coordinator

NMFP Transition Coordinators will provide you with information about eligibility requirements and services for for NMFP and will also provide an overview of services available through the Aged and Disabled Waiver and the Developmental Disabilities Waivers.

If you meet the eligibility requirements for NMFP, a Transition Coordinator will refer you to a HCB Aged and Disabled Services Coordinator or a Developmental Disabilities Services Coordinator who will determine whether you are eligible for one of the home and community-based waivers. If you do not qualify for the NMFP project, the Transition Coordinator will make appropriate referrals as needed. If you qualify, your HCB Services Coordinator will then assess your needs and the supports needed for living in the community. HCB Services Coordinators will work with you to develop a transition plan.

NMFP Transition Coordinators will maintain contact throughout the first year of transition to make certain that your transition is successful. They will also perform a Quality of Life Survey prior to your transition and at the end of each of the first two years to determine the success of your transition and to document accomplishments as well as challenges to your transition.
