Care Management

Care Managers assist older persons and their families understand the functional limitations faced by the older person and connect them to a variety of community support services. This process includes an assessment of the individual and the development of a plan of care. After a discussion of the assessment and the plan of care, the client selects their priorities. The Care Managers will arrange the support services requested. Care Managers will follow the individuals and adjust the support services as their needs change.
