Disabled Persons and Family Support
 
The Disabled Persons and Family Support (DPFS) Program provides state-funded assistance to individuals of all ages who meet specified income and disability criteria. The program assists people with disabilities to remain employed, maximize their independence, and remain in their homes with their families.
 
Eligible individuals may receive funding up to $300 a month or $3,600 annually for services such as home delivered meals, personal care, incontinence supplies,  housekeeping, transportation, special equipment, and vehicle or architectural modifications, commercial travel and lodging. The Program does not operate as an entitlement and services are intended to supplement, but not reduce the responsibility for the services and supports available through other programs for which the family or disabled person is eligible such as Medicaid, Social Services Block Grant or other programs with Federal funding.
 
The Disabled Persons and Family Support (DPFS) Program is based upon state law.   The program serves three populations:
  1. Families who provide for a family member (either adult or child) living with them who has a disability and needs some form of support to keep the family together and prevent out-of-home placement;
  2. Persons who are disabled and employed (or could be employed), earning at least $500 per month, who need some form of support to maintain employment;
  3. Persons who are disabled and living along or with a non-relative caregiver to maintain the independent living situation.
Persons may be financially eligible if their gross monthly income adjusted for family size does not exceed the following:

In addition to financial and need eligibility, Disabled Persons and Family Support requires medical information using a Disability Report completed by a healthcare professional which accompanies the application to verify medical need(s).
