Nebraska contracts with U.S. Bank to provide the ReliaCard Visa for you to receive and spend your State payments. This card gives you more convenience, security and purchasing power than a State check.
You complete the FA-100-ES-C form to request the ReliaCard Visa
DHHS receives and processes your form and sends the information to U.S. Bank
U.S. Bank mails the card to you in a plain white window envelope. This envelope also contains four tools to give you card details and help you begin using the card.
For lost or stolen cards, call (855)233-8382. (The balance from the old card is transferred to the replacement card. Future payments will be made to the newer card.)
When your ReliaCard Visa arrives in the mail, follow these four steps to get started:
Call (855)233-8382 to activate your card and select your Personal Identification Number (PIN). Once activated, all future payments will be deposited onto the card and are available immediately.
Sign the back of your card in ink. Your card is not valid unless you sign it.
Use your card to make purchases, pay bills or withdraw all of the funds, however, only the State can make deposits.
Track your purchases and balances. Don't forget to include cash withdrawals, miscellaneous fees, purchases, and State deposits.
