The Quality Assurance Team completed the fourth round of Initial Safety Assessment Reviews in the Fall 2010 through the Spring 2011. The period under review was October 2009 – March 2010. The QA Team completed 638 reviews out of the 5400 (11.8%) eligible Initial Safety Assessments documented in N-FOCUS for the State during the period under review. The report also includes some reviewer comments and observations. Charts and tables containing data from the reviews are attached to the report.

  The Nebraska Safety Intervention System was implemented in the panhandle area of the Western Service Area (WSA) first and then in the eastern part of the WSA.   As a result, reviews are conducted and reported separately for the two areas.
