  • Severe asthma
  • Serious burn injuries
  • Cerebral palsy
  • Craniofacial disorders (may include cleft lip, cleft palate, and other facial defects)
  • Cystic fibrosis
  • Diabetes
  • Eye conditions requiring surgery
  • Significant hearing loss
  • Heart conditions
  • Hemophilia
  • Midline neurological defects
  • Neoplasm including leukemia and lymphoma
  • Neurological conditions, seizures
  • Orthopedic conditions (may include muscles, joints, ligaments, or spine)
  • Juvenile rheumatoid arthritis
  • Urology conditions
