What you need to know

Submitting a Claim in 4 Steps

  1. Login to For Registration instructions, go to the Create a User Account Page
  2. Click on View Open Claims

  3. Fill out the fields you wish to search by and click Search

  4.  Fill out the claim information and click Submit
